This is a Microsoft Office File that is used to obtain approval for the removal and use of University property off premises.

The Dean, Director, or Department Head must sign this form. A copy should be filed with the department’s Property Coordinator.

The Authorized User should retain a copy of this form while transporting equipment to and from campus. SECURITY MAY REQUEST TO EXAMINE THIS AUTHORIZATION FORM WHILE EQUIPMENT IS IN TRANSIT.

The Authorized User must notify Financial Analysis when the equipment is returned to campus.
