The purpose of the Journal Entry form is to allow authorized campus users the ability to request a journal entry to move existing (actual) financial transaction(s) from one Chartstring to another. This form is to be used ONLY for journal entries that involve a Sponsored Chartstring, or for entries that require submission of supporting backup documentation.
The form is to be filled out by individual(s) who have been authorized by the department(s) for transaction approval and submitted with appropriate supporting backup to:
